Our Services

Skilled clinical care, at home

We are a Medicare-certified home health agency. Our clinicians bring hospital-level skills into the home, so recovery can happen where people are most comfortable.

Skilled nursing

Registered nurses providing assessment, treatment, medication management, and education — with primary care support from our Clinical Director.

Advanced wound care

Expert management of complex wounds, including negative-pressure wound therapy, to promote healing and prevent complications.

IV therapy & nutrition support

Care for patients receiving intravenous therapies — TPN, antibiotics, cardiotonic therapy — and safe support for enteral nutrition at home.

Physical therapy

Rehabilitation with deep experience in joint-replacement recovery, helping patients regain mobility, strength, and confidence.

How we work

Nurse-owned. Nurse-led.

Primary HomeCare Specialties is owned and operated by nurses, and it shows in how care is organized. Our model is built on continuity and accountability, so every patient knows exactly who is responsible for their care.

  • Every admission starts with our Nurse Director. She personally conducts each initial assessment and oversees the admission, working alongside the assigned nurse or therapist.
  • One clinician, start to finish. Each clinician manages their own caseload, so patients see the same familiar face throughout their care rather than a rotating cast.
  • A coordinated team behind them. Nursing, therapy, and the Clinical Director work as one team, keeping care seamless and tailored to each person's needs.
